Welcome to the S-SV EMS Online Application Center. This page contains application instructions, required forms and links to secure/encrypted online applications for all levels of EMS personnel.
Application fees and payment instructions are listed on the individual instructions documents as well as on the confirmation page after you submit your completed online application. You can also click here for a schedule of current application fees. Money orders or debit/credit cards are the only accepted forms of payment for all application fees. Please contact us if you need assistance, have questions, or to request a written application.
Note: CPR, ACLS, PALS and PEPP courses must have an instructor led, hands-on classroom skills component for the certification to be considered valid. Certification cards issued for online only courses with no hands-on skills component will not be accepted. In some instances, applicants may be required to provide additional proof that the course(s) included the required instructor led, hands-on classroom skills component.
EMT/AEMT Initial Certification
EMT or AEMT applicants who do not have a current or lapsed California EMT or AEMT certification card issued by the S-SV EMS Agency.
Paramedic Initial Accreditation
Paramedic applicants who do not have a current or lapsed paramedic accreditation card issued by the S-SV EMS Agency.
Emergency Medical Responder (EMR) Certification
EMR Initial Certification
EMR applicants who do not have a current or lapsed EMR certification card issued by the S-SV EMS Agency.
Mobile Intensive Care Nurse (MICN) Authorization
MICN Initial Authorization
MICN applicants who do not have a current or lapsed MICN authorization card issued by the S-SV EMS Agency.
