MICN Reauthorization Application Confirmation

Your S-SV EMS MICN Reauthorization application has been completed. The following additional requirements must be met before your MICN Reauthorization can be issued:

  • Submission of the following supporting documents (uploaded with your online application or emailed to info@ssvems.com).
    • Copy of your current California State RN License.
    • Copy of your current U.S. state-issued Driver’s License or photo ID.
    • Copy of your current CPR certification/recognition.
    • Copy of your current ACLS certification/recognition.
    • Copy of your current PALS/APLS certification/recognition.
    • Employment/recommendation letter from an S-SV EMS Base Hospital.
  • Payment of the MICN Reauthorization fee.
    • Click on the ‘Pay Fee Online’ button below to pay the $104.00 fee with a credit/debit card ($100.00 reauthorization fee + $4.00 online payment processing fee), or mail/deliver a $100.00 Money Order to: S-SV EMS. 535 Menlo Drive, STE A, Rocklin, CA 95765.
