S-SV EMS now offers 2 convenient ways to certify or recertify your EMT / AEMT.

1. EMT / AEMT Online Application (Certification and Recertification)
   Download the DOJ Livescan form here
   
   Please note: Application materials not submitted through the online application, such as card copies and DOJ Livescan form, MUST be delivered to S-SV EMS in order to complete the application process.


2. Download the EMT/AEMT Certification / Recertification materials provided here and mail or hand deliver them to SSV-EMS.

EMR

S-SV EMS now offers a convenient way to certify or recertify your Emergency Medical Responder (EMR).

1. First, click here to download a "zip" or compressed file.
2. When the file downloads, uncompress or "unzip" it. Usually simply double-clicking the zip file will uncompress it.
3. You should now see a folder with the following documents (4 total):
   • EMR Application.pdf
   • EMR Instructions.pdf
   • EMR Skills Form.pdf
   • S-SV EMS EMR Live Scan Form.pdf
   
   
4. Read the instructions thoroughly!
5. Mail or Fax documents to S-SV. You may also email completed documents to certification@ssvems.com
